The Apollo 8 spacecraft, CSM-103, has been removed from the altitude chamber in the Manned Spacecraft Operations Building at KSC on September 24, 1968, and is prepared for installation of the SPS engine nozzle. Note that one panel on the service module has yet to be installed.

That looks to be the same panel that blew out on Apollo 13.
