From what I have seen, there was only one LM equipment jettison planned for the Apollo 13 mission. That would have been after EVA-2.

The Soyuz 11 crew all died because they were not wearing suits when their capsule lost its air. If they had been wearing suits* it would be ridiculous to suggest that the loss of air created an EVA scenario, yet that is the logical consequence of the Russian definition, as quoted.
* I acknowledge that the Soyuz 11 capsule was too cramped to carry three suited cosmonauts, but that does not negate the point.

The definition of an EVA with a crew member exposed to vacuum evolved throughout the years and definitions changed somewhat regarding the semantics of what constituted an EVA for history.
