I am trying to identify the item below, I am told it is a mirror flown on Apollo 13, but I would like opinions based on the serial numbers, etc.

It is definitely an Apollo Command Module mirror. But I don't think there is enough evidence to say whether it is flown or not.

Very nice item, Mike.

Take a look at NASA image number AS07-04-1584 which shows Walt Cunningham during Apollo 7 with what I believe is this type of mirror on the top left of the shot.

I was looking at that image today and just noticed the mirror in the frame. Given some time, I am sure even better shots of those types or mirrors in flight can be produced.

Hope this helps in some way.
